  • The effect of ultrasound with acoustic radiation force impulse on lung tissue in large animals

    音響放射力インパルス(ARFI)は組織の硬度を測定する超音波検査(ARFIエラストグラフィー)として近年臨床使用されているが、我々はこれまでに動物実験においてARFIの照射により意図しない肺損傷が起こる可能性を報告してきた。ARFIエラストグラフィーの安全性を検討するため、今回、臨床使用に近い条件でARFIの肺への照射実験を行った(本実験は文部科学省における「研究機関等における動物実験等の実施に関する基本指針」に従い、自治医科大学動物実験委員会の承認(承認番号17233-01)を得たものである)。方法としてはカスタムメイドしたARFI装置(Bモード表示部とARFI送信部をスイッチで切り替え可)を使用し、リニアプローベ(5.2MHz)をウサギの左右の上腹部肋弓下の体表に設置し、Bモードで肺表面を描出、照射部位を決定、一時的に呼吸を停止させた上で同部位に照射を行った。照射の条件はPulse duration (PD): 0.3ms, focus: 30mm、照射回数: 30回、照射間隔: 3sとし、Mechanical Index (MI)を6段階に設定(①0.37 ②0.49 ③0.58 ④0.75 ⑤0.84 ⑥0.88)した。各々のグループのサンプル数は6とした。結果は肉眼所見としては照射した部位に一致して肺表面にred spot(病理組織所見では照射部位の胸膜直下の肺胞出血)を認め、得られたデータからはMI値と肺損傷(red spotの発生率及びその面積)の間には正の相関を認めた。本実験は動物実験であり、同部位へのARFI照射回数が30回と多いものの、FDAの認証基準であるMechanical Index < 1.9 において肺損傷(肺胞出血)が確認されており、今後さらなる検証が必要と考えられた。, Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, 2019/04 -2023/03, Grant-in-Aid for Scientific Research (C), Jichi Medical University

  • Quantitative diagnosis of non-alcoholic steatohepatitis by ultrasound and elucidation of the mechanism of occurrence by ultratrace mass spectrometry

    A new system to elucidate the pathogenic basis of non-alcoholic hepatitis from the metabolic aspect in detail using a medaka model is developed. Specifically, the main bile acid of medaka, which is rare for fish, is C24 (TCA) and C27 (THCA), and succeeded in visualizing the distribution of these two bile acids within one medaka individual. In addition, the main component analysis revealed the site and metabolism of each digestive organ. Regarding the relationship It was suggested that these two bile acids are functioning.   • Biosafety for the ultrasonic wave ---Genetic analysis stepped over the generation ---

    Quantitative references for the ultrasonic diagnosis were discussed from the viewpoint of biosafety. Experiments were carried out throughout the ultrasonic wave irradiation on living body with paying the attentions to "changes between a gene expression and a protein expression ". As a kind target for the observation, Medaka(Japanese killifish ) was applied because of the short generation of three months, then the genetic effects were also observed. As a result of performing the ultrasonic wave irradiation to a Medaka embryo, and having performed proteome analysis, AHCY enzyme was identified as a change protein. In addition, as for this enzyme, it was observed the increase of the quantity of outbreak with a rise of ultrasonic sound pressure.   • Three dimensional blood flow vector measurement by using ultrasonic crossed beam contrast echo method

    Since the Doppler frequency of summed and different frequency observed by the ultrasonic crossed beams contrast echo method is calculated as a function of two angle components for the beams to the flow direction, a blood flow vector is estimated by solving the simultaneous equations of second harmonic and summed components without beam angle. This study discusses the feasibility of the proposed method by using experimental system incudes flow phantom. This experimental system is constructed as follows. The silicone tube of 4mm of inner diameter is located in the degassed water tank. The two ultrasonic beams of frequency of 3.2MHz and 2.0 MHz are crossed at the tube. Microbubbles (Sonazoid) are flowed in the tube with degassed water. Flow velocities are set as 10 mm/s, 20 mm/s and 30 mm/s. The summed frequency is 5.2MHz and second harmonic is 6.4 MHz.     This study concentrates on the improvement of image degradation of ultrasonic echographic images by using higher order harmonics which are generated while ultrasound waves propagate through the biological tissues., Since image degradation caused by speckle patterns in the echographic image has been a problem, it is an important task to reduce these patterns to obtain higher quality images. In order to detect the higher harmonic components contained in the echo signals efficiently, a specialized transducer for broadband reception, besides the transmitting transducer is necessary. By constructing a mechanical sector scanner of an ultrasonic diagnosis system using such a PZT-PVDF two-layered ultrasonic probe, an ultra-broadband ultrasonic imaging system capable of a real time imaging was prototyped. Thus, the real time imaging was carried out by obtaining the echo signal from phantoms using a PZT-PVDF two-layered ultrasonic mechanical sector probe, and the effectiveness of its higher harmonic components from the fundamental to the fourth harmonic was confirmed in speckle reduction. Then we prototyped the novel transducer which transmits ultrasonic pulse of dual-frequency and receives multi-frequency echoes from the biological tissues. When the ultrasonic pulse off and 4f_0 are transmitted into the biological tissues, 2f_0, 3f_0, and 5f_0 components are generated as the secondary waves. We have prototyped the PZT disk of 7 mm in diameter coaxially arranged in the PZT ring of 17 mm in outer diameter The ring and circular transducers transmit the pulses of 2 and 8 MHz, respectively. It was confirmed that this transducer formed the ultrasonic beams of 4, 6 and 10 MHz as well as the beams of 2 and 8 MHz in degassed water. Then the experiments of imaging the agar-gel phantom and pork meat were carried out by mechanical scanning.   • Ultrasonic Real-time Imaging Using Acoustic Fourier Transform

    The purpose of this research is 1. Development of the new method that performs Fourier Transform of the object by the ultrasound (Acoustic Fourier Transform) and reconstructs the object by Inverse Fast Fourier Transform in the computer. 2. Experiment of reconstructionabout the glass, the stainless steel, which are transparent by the optical method, and the object in the flame, the mud, the mist, which is called the ultimated environment, for the verification of this method. We made the reflection type Acoustic Fourier Transform system. An object and a reference source are arranged in the front of the transmitter-receiver array which arranged many receivers in the circumference of one transmitter. Here, these elements are arranged sot that they may be on the same perimeter. Then, Fourier Transform of the object can be obtained at the receiver array by interference of the reference wave and the reflection wave from the object (the object wave). Moreover, in this method, it was decided with the system that the reference signal made in the computer was electrically added on the circuit for the substitute of arranging a reference source in the neighborhood of the object. And, automatical focusing, which measured a distance to the object by the pulse echo method, was included to adjust the delay quantity of the reference signal made in the computer. Using this system, we reconstructed the object (geometrical easy figures, alphabet characters) made of the acrylic fiber as an example of the optical transparent object, and the object in the smoke as an example of the object under the ultimated environment. As a result, each object was reconstructed sufficiently, and the validity of this method was confirmed.   • Automated Measurement of Organ Volume Using Ultrasonic Bubble Filling Method

    The purpose of this study is to measure the volume of the target organ accurately and automatically from ultrasound images. In order to measure the volume accurately, it is necessary to determine the boundary of the target organ beforehand. It is, however, difficult to detect the organ boundary satisfactorily, because the ultrasound image quality is seriously deteriorative. Therefore, we developed the method for measuring the volume of the target organ accurately and automatically using the newly proposed Bubble Filling Method. As a result of applying this method to agar gel phantom data, it was possible to measure the volume accurately and automatically. In this research period, we measured 50 fetal bladder volumes to verify the method's effectiveness. Data acquision was as follows : the angle (parallel) and the speed of the probe applied to the mothers' abdomen were fixed during the scanning. Then we applied this method to the data and measured the volume. As a result, it was possible to measure the volume accuraterly and automatically. Finally, we proposed the method for measuring the volume of urinary bladders accurately and easily from ultrasound images. A set of two-dimensional cross-section images and a vertical-section image are acquired by parallel scanning and then the urine bladder volume is measured accurately without special hardware. As a result of applying this method to experimental data and clinical data, it was possible to measure the volume of the urinary bladder.
